- Step #1: RPCs will receive Security Assessment Questionnaire (SAQ) email as shown below. Click on the
icon to begin.

Next, you will be prompted to set up a password.
Please note the password tips. Passwords must be more than 8 characters and must contain numbers and specific special characters (! @ # % ^ & *). Please do not include any part of your login name and avoid words in the dictionary. Your username will be your official work email address.
Step #2: Once a password is established, you can access Qualys SAQ. You will see the name of your PSAP locations, the state of the questionnaire (not started), progress by percentage (50%), Date you received the questionnaire, and the Due Date.
You will have up to two weeks to complete the questionnaires.
Next, to open the questionnaire, click the drop-down 
arrow as shown below. Select 
.
Below is an example of one of the 22 questions from the Campaign 1 questionnaire.
Please note Qualys accepts most document formats such as ixls, jpeg, png, pdf etc. However, the size limitation for each uploaded file is 2MB.
After you complete the questionnaire, select submit and it will disappear from the list. Please remember to log out of the system.
